Currently our Medical Providers are accepting the following commercial insurance plans:
Our practice does not accept Medicare, Medicare Replacement Plans, Medicare Supplements, or Medicaid.
Our mental health providers are accepting Aetna and BCBS commercial insurance.
At this time we are not pursuing credentialing with any other insurance companies for our mental health providers. We will keep you updated should this change in the future.
Our new mental health provider is working towards BCBS and Aetna credentialing at this time. A superbill can be provided if needed.
$300 – New Patient Visit
$150-240 – Follow Up Visit
$150 + $35 per pellet – Hormone Pellet Insertion
For medical visits the cost may vary depending on the appointment type you have with the medical provider, this is not a complete list of charges or procedures. Labs are processed directly from the lab company and are separate charges. You will receive a separate bill from the lab.

Currently, SoftWave Therapy is not reimbursed by insurance and is offered as a self-pay service. However, you may use FSA or HSA funds for payment, and we’re happy to provide a superbill should you wish to submit a claim independently.
The number of shocks depends on the treatment location and protocol. Most treatment protocols recommend a minimum of 1500 pulses per SoftWave therapy session to achieve optimal results.
